Scoring Guide (0-4):
0 = Not present
1 = Mild
2 = Moderate
3 = Severe
4 = Very severe

1. Anxious mood (Worries, fearfulness, irritability)

2. Tension (Fatigability, restlessness, trembling, inability to relax)

3. Fears (Darkness, strangers, being alone, crowds, animals)

4. Insomnia (Difficulty sleeping, nightmares, night terrors)

5. Intellectual (Difficulty in concentration, poor memory)

6. Depressed mood (Loss of interest, lack of pleasure, early waking)

7. Somatic (muscular: Pains, aches, stiffness, twitching, grinding teeth)

8. Somatic (sensory: Tinnitus, vision blur, hot/cold flushes, weakness)

9. Cardiovascular symptoms (Palpitations, chest pain, fainting, racing heart)

10. Respiratory symptoms (Shortness of breath, chest tightness, choking)
